| Utility Failures - Are You Prepared? | ||
| Katherine E. Pearson, Risk Management Services, Rohm and Haas Co., 6519 State Hwy 225, Deer Park, TX 77536 Many incidents have been triggered by unanticipated interactions with utility systems. Utility systems have many connections to processes, both physical and electrical. This interconnectivity results in common mode failures and the potential for cross-contamination. This paper will use “real-plant” incidents to illustrate what can happen when the hazards of interactions are not identified or understood. Common failures which occur in utility systems will also be discussed. Extended Abstract Status: File Uploaded | ||
