478362 Effective PSSR in Management of Change for an Existing Plants

Sam Sanati, APEGA, Calgary, AB, Canada

Management of Change (MoC) and Pre-start up Safety Review (PSSR) are key elements in OSHA process safety management (PSM).

However, in complete or incomprehensive PSSR is one of the root causes in the most of the incidents occured in industry. Although, PSSR is being satisfacory focused in the green filed projects, it may not be completely followed during operation in existing plants. Some key contributors to neglect PSSR are:

- PHA action items are not fully closed and implemented

- Process Safety Information (PSI) are not finalized and communicated

- Lack of Training

- PSSR has not been reviewed thoroughly

This preseintation demostrates collected experience during actual PSM audits and emphasizes on main the elements of PSSR. It would also assists the operating plants to conduct thorough PSSR prior to energize or activate the process.
