Goals/Purpose

:

Upper face aging presents as sagging and volume change of eyebrows and temples , and also appearance of static and dynamic wrinkles in forehead.

In this study, an advanced foreheadplasty technique through upper blepharoplasty is introduced. In other words , whatever techniques in

endoscopic forehead lift is simulated through a minimal access trans-palpebral approach.

Methods/Technique

:

A cohort of 54 patients experiencing age related changes of eyebrows and forehead underwent the trans-palpebral foreheadplasty between February 2020

to October 2022. This advanced technique involved predetermined upper lid skin excision , creation of orbicularis muscle window , supraorbital rim exposure

, neurovascular bundle skeletonization, supra orbital periosteal cut, completely dissected frontal and temporal pocket. Dynamic part of foreheadplasty is performed

through excision /division of Corrugator supercilli, deppercilli , procerus, orbicularis oculi muscles.

Results/Complications

:

Transpalpebral foreheadplasty significantly raised the eyebrows at all predetermined points for each eye.

This elevation effect was maintained significantly for more than 18 months. Degree of softening of wrinkles

was statistically significant and correlated by extent of muscle action.

Conclusion

:

The transpalpebral foreheadplasty emerges as reliable and effective method with low risk of complication.

The outcome of this minimal access technique is comparable to endoscopic foreheadplasty as standar

level of care.
