Did you know that your brain thinks in 2 different ways? Fast and slow? And that if you use fast thinking too much, you will be blind to critical anomalies; or mental bias will overcome critical thinking. Hence: incident during startup or issues not registered by AVO. What if we had thought of the critical information while trouble shooting our plant?
