Treatment of Sunken Upper Eyelid Deformity in Elderly Asian Patients: Blepharoptosis Correction with Medial Orbital Fat Pad Transposition and Orbicularis Oculi Preservation
This study aims to report the outcomes of treating sunken upper eyelid deformity in elderly Asian patients through blepharoptosis correction using medial orbital fat pad transposition and preservation of the orbicularis oculi.
Methods/Technique:
From June 2017 to July 2024, we treated 153 Asian patients over the age of 40 with sunken eyelid deformity. Surgical procedures included the advancement of the levator aponeurosis and Müller’s muscle complex. The medial orbital fat pad, which is often prominent in elderly patients, was transposed to the central area to enhance aesthetic results. During dissection, the orbicularis oculi was incised along the lower flap margin and preserved to the upper flap to increase the volume of the upper eyelid.
Results/Complications:
Satisfaction with the treatment was reported in 78% of patients (120 cases) according to a subjective survey. Eleven patients experienced poor outcomes and required additional procedures. Symptoms related to eye opening movement improved in nearly all patients.
Conclusion:
Blepharoptosis correction utilizing medial orbital fat pad transposition and orbicularis oculi preservation effectively addresses sunken upper eyelid deformity in elderly Asian patients.
